Leaving the Hospital

When it’s time to leave St. Bernards Medical Center, we help you transition safely and smoothly to your next destination. Our goal for this process—called hospital discharge—is to provide all the information you need to take care of yourself. A nurse will meet with you to review information such as:

  • Follow-up appointments at a location that’s convenient for you
  • Medications you were prescribed
  • Medical equipment you may need for your recovery

We’ll answer your questions and give you written instructions to take with you. This process may take place in our discharge lounge. Relax here instead of your hospital room while waiting for your ride to arrive.

When is Discharge?

Discharge planning starts on day one. Within two hours of your doctor’s discharge order, we want to help you be ready to leave the hospital. Most patients leave by 5 p.m. the day they’re discharged.

If you don’t have a patient portal account, call the St. Bernards Medical Records office at 870.207.4169 for help signing up. Through this secure website and mobile application, you can:

  • View your discharge summary and other health records
  • See upcoming appointments and request visits
  • Send a message to your provider
  • Request prescription refills

We’ll make sure you have support to keep recovering at your next destination—whether that’s home (with home health services), a nursing facility, a rehabilitation hospital or another one of our locations.

You can call 870.207.7488 to schedule a free Nurse by Appointment session to discuss discharge instructions and medications after you return home, if needed. Appointments are available Monday to Friday, 8:30 a.m. to 3:30 p.m.

Outpatient Rehabilitation

With facilities throughout northeast Arkansas, St. Bernards offers physical therapy, occupational therapy and speech therapy close to home. If your doctor orders rehabilitation, we’ll make a referral to a location near you so you can start to regain your abilities after an injury or surgery.

Follow-Up Call & Survey

After you leave the hospital, you’ll receive a phone call asking about your experience and giving you a chance to ask questions. You’ll also get a survey in the mail or by text message. Share your constructive feedback to help us improve the patient experience.
