Population Health
Good healthcare offers more than treatment for an illness. It also helps you access the information and resources you need to stay well. That’s why St. Bernards Healthcare offers primary care, education, case management and other preventive and supportive services. They’re designed to meet you wherever you are on your journey—and help you take the next step toward better health.
Invite Us to Visit Your Group
Let St. Bernards help make your school, office, church or other organization a healthier place. Invite a St. Bernards clinician or educator to:
- Speak to your group on a topic of your choice or lead a health screening event
- Train members of your faith community to visit and support each other after a hospital stay as part of our Congregational Health Alliance

Congregational Wellness
St. Bernards Congregational Wellness is a collaboration between area congregations and St. Bernards Healthcare to impact the health of our community. The purpose of congregational caring is to give hope to those who need it. Regardless of your hospital affiliation or church denomination, we can help your members navigate challenging healthcare environments and make it easier for them to stay healthy. Because spiritual, physical and mental health work together for total wellness, we focus on health awareness, disease prevention and educating on disease management phases.
CPR & Life Support Training
Help save lives in your community. Call 870-207-4988 to ask St. Bernards to train your group in these American Heart Association programs:
- Heartsaver Cardiopulmonary Resuscitation (CPR)
- Basic Life Support (BLS)
You can choose from in-person training or a blended option that includes online learning and a hands-on skills session.
Value-Based Care Programs
Your primary care doctor at St. Bernards participates in value-based care agreements with Blue Cross Blue Shield, Humana, United Healthcare, Centene, Aetna, Wellcare, Medicaid and Medicare. Medicare and Humana are part of an accountable care organization. The remaining insurance providers are part of a value-based care program. This program makes agreements with insurance providers to be financially accountable for the quality, cost and experience of care you receive.
For you, value-based care agreements mean:
- More communication among your healthcare providers, so you receive the right services at the right time
- Less likelihood of unnecessary medical tests and appointments
- Access to a dietitian, CPR training and employee navigators
- Improved quality of care
- Better patient outcomes
Case Management
Because value-based care agreements give clinics extra incentives to keep you healthy, your primary care office may also provide case management services. Case managers:
- Help you arrange all the services your doctor orders
- Remind you of appointments, tests and treatments you need
- Explain your diagnosis, treatment plan and hospital discharge instructions
Diabetes Management
Controlling diabetes is one of the best ways to reduce your risk of other illnesses, including heart disease, stroke and blindness. To take charge of your health, ask your doctor for a referral to St. Bernards’ diabetes education program. You can also check our events calendar for upcoming meetings of our free Diabetes Management Support Group.
Nutrition Counseling
Take charge of your health with nutrition counseling from a registered dietitian at St. Bernards Healthcare. You’ll receive guidance tailored to your nutritional needs for conditions such as diabetes, heart failure, kidney failure and more.
Patient Centered Medical Home
Arkansas Medicaid recognizes Paragould Doctors Clinic as a Patient Centered Medical Home. That means this family medicine office:
- Provides detailed care plans and extra support to patients at the highest risk of health problems
- Makes care available to all patients 24/7 through an after-hours phone number
- Delivers health instructions and educational materials in a way patients can understand
- Involves your family in your care as needed
Community Health Assessment
Every three years, St. Bernards reviews the health of the communities surrounding our hospitals in northeast Arkansas. These evaluations help us determine what services you and your neighbors need most. To learn more, read our community needs assessments.
