Failure to Thrive Evaluation and Management
“Failure to thrive” means an older adult struggles with performing normal, day-to-day tasks necessary for independent living. Its symptoms include frailty, weight loss, lethargy, recurrent infections, depression and cognitive decline. Failure to thrive needs timely intervention to reverse its pathway. Our clinic helps with chronic illness and medication management while evaluating and managing mobility issues and mood and cognitive disorders.
Geriatrics Home Visits
Many of our patients are physically frail, have mobility limitations or experience memory loss. We can address their medical needs at home, boosting their quality of life. Our home visits include physical exams, treatment of acute and chronic medical problems, medication management, lab tests and more.
Geriatrics Inpatient Services
If you are admitted to acute rehabilitation at Encompass Health Rehabilitation Hospital of Jonesboro or the geriatric unit at St. Bernards Behavioral Health, our team brings specialized geriatric care to you while you are in the hospital.
SAFE (safety evaluation and fall prevention) Program
We evaluate your risk of falling related to general conditions, medications and normal changes of aging. In addition, we collaborate with physical therapists at St. Bernards Villa to offer you a complete safety and fall prevention program.
Pain Management
We offer you with complex pain evaluation and management options for arthritis, cancer related or neuropathic pain. We also work with physical therapists and regular pain management clinics to adjust your treatment as needed.
Information
We have a large resource library and skilled social workers who can get you information about housing needs, home aids, medication support and medical insurances.
Decision Support
We provide counseling regarding advanced directives and other services relating to palliative and quality-of-life oriented care.
