Home Health is a cost-effective, quality-driven alternative to hospitalization or a nursing facility. Our healthcare professionals provide the necessary care and medical oversight to maintain or restore health in the comfort of your own home.
St. Bernards Home Health provides skilled and non-skilled services in patient homes. Services include skilled nursing, personal care, physical, occupational and speech therapy, and social services.
These services are offered 24 hours a day, 7 days a week, within a 50-mile radius of St. Bernards. This can be on an hourly basis for greater needs or through regular scheduled visits.

Meal Delivery
Meals Express is a convenient meal delivery service supplying area residents with proper nutrition. The program is designed to supplement recipients’ regular meals. Meals are available in a two-meal or a five-meal pack, delivered once a week to the door or freezer. All meals are prepared under U.S.D.A. inspection and are planned to meet or exceed the recommended dietary allowance of calories, vitamins and minerals.

Lifeline Personal Help Monitor
Lifeline is a personal response service with year-round, 24/7 monitoring by trained professionals. If you need medical assistance or other assistance, help is available with the touch of a lightweight, waterproof button, worn as a pendant or wristband. A certified Lifeline representative will respond within seconds, assess the situation and summon the appropriate help.
A Lifeline home unit is easily connected to a standard home phone jack by a trained St. Bernards staff member. The comfort of knowing help is always near is available for a little more than a dollar a day. For more information, please call 870.207.7635 or toll-free 888.325.4680.
Total Life Healthcare
Total Life Healthcare is a managed care program providing a comprehensive set of healthcare services to individuals 55 and older who can safely reside at home but need long-term care services in order to do so. The program utilizes the PACE (Program of All-Inclusive Care for the Elderly) model to ensure care and services are coordinated to support program participants, family members and other caregivers.
St. Bernards and the East Arkansas Area Agency on Aging have partnered to create an all-inclusive elderly care program that includes these services and many others:
- Transportation
- Home Health
- Geriatrician, primary care and specialty physicians
- Hospital care
- Adult daycare
- Pharmacy
- Physical therapy
- Speech Therapy
- Meal delivery
- Lifeline personal help monitor
- Social work
Total Life Healthcare is provided at no cost for participants with Medicaid or Medicare with Medicaid. The program is available for residents living in specific ZIP codes within a seven-county service area.

DayPlace Adult Day Care
DayPlace is a safe, dependable care option for senior adults who should not be left alone during the day. The program encourages independence and companionship while providing families with peace of mind.
Care hours are Monday-Friday, 7:30 a.m. to 5:30 p.m. Participants are served nutritious meals and snacks as part of their stay. As a nurse-directed program, staff can provide personal assistance, administer medications and monitor health problems, as necessary.
DayPlace is a private, non-profit program that is a Medicaid Elderchoice provider. Out-of-pocket fees are $12.48/hour with medications administered and $10/hour without medication assistance. For more information, please call 870.207.7520.
